Our role at Aesthetic Imaging is that of a business associate to your physician’s practice. In this role, we strive to safeguard your non-public personal information with the same high level of diligence and security, as does your physician’s practice. This privacy policy details our commitment to protect your non-public personal information.

We maintain commercially reasonable protocols to ensure the security and confidentiality of your non-public personal information to prevent unauthorized access to the information we store for you. We have physical security in our building, passwords to protect databases, compliance audits, and virus/intrusion detection software. With our service, access to your information is limited to you, your physician and to those within Aesthetic Imaging who need such access to do their job. Unfortunately, no data transmission over the Internet can be guaranteed to be 100% secure. As a result, while Aesthetic Imaging takes all commercially reasonable steps to protect your non-public personal information, your use of this site is at your own risk.

Your records will not be released to anyone or any organization without your express written consent, your guardian’s consent or by court order. We will never disclose your non-public personal information for promotional purposes and we do not sell your information to third parties.

You may revoke your consent to the posting of your non-public personal information in writing at any time. Your consent to the posting of your non-public personal information on this site has no bearing on your medical care.

• Grant permission to Aesthetic Imaging to place your personal treatment photographs (your “photo library”), provided by your physician, onto a secure, password protected, online database. Aesthetic Imaging makes all reasonable and expected efforts to maintain the security of this database.
  
  
• Agree to hold Aesthetic Imaging, its employees, its owners, its agents, your physician, your physician’s employees and physician’s agents harmless and not liable from any and all losses, claims, damages, liabilities and obligations of any kind and description, including any reasonable attorney fees, arising out of any matters related to the photo library service, including, but not limited to, any breach of security, regardless of fault, and any liabilities related to Aesthetic Imaging’s operations as a contracted business associate of your physician.

This privacy policy and disclaimer only addresses the use and disclosure of your non-public personal information by Aesthetic Imaging. If you disclose information to other parties, different rules may apply to their use or disclosure of such information regardless of their affiliation or relationship with Aesthetic Imaging. Because Aesthetic Imaging does not control the privacy practices of third parties, you are subject to their privacy customs and policies.

Password Protection

You will be provided with a password to access your non-public personal information. You should not disclose your password or share it with any third parties. If you lose control of your password, you may lose control over your non-public personal information. Therefore, if your password is compromised for any reason, you should immediately notify Aesthetic Imaging so that we may issue you a new password.
